How often do you finish up a therapy session with the best of intentions to practice whatever you spoke about with your therapist only to remember as your next appointment starts that you didn’t do it?

How many times have you had a realization or reaction to something your therapist said and wanted to remember it later only to find that the memory is fuzzy?

Many times over the years I’ve seen client scramble for a post it or receipt to take down their own notes in the middle of a discussion, and as someone who often uses journal layouts to organize my thoughts and activities I wanted to share a way to write down any notes that you may want to keep from a therapy session.
